You may apply for Visa and Social Visit Pass to enter and remain in Singapore for a defined period of stay to seek medical consultations and treatments. A local sponsor (i.e. a Singapore registered corporation / Singapore citizen / Singapore permanent resident who is 21 years old and above) is required. There are several forms you need to fill in and submit together with other necessary documents. Please make sure that all the forms are duly completed and signed and all the required documents are in English language (if not, please include notarized true copies of English translation). Your sponsor may also need to lodge a security deposit if requested. If you are seeking medical treatment of serious illness (e.g. cancer) in Singapore, you may submit your application by proxy. All original documents must be presented for verification.
Documents to be prepared:
1. Original copies of duly completed Form 14, Form V39I* and Form V75 (Form V39I is only for non-Malaysians). Form V75 can be replaced by a letter from a registered doctor in Singapore stating brief details of the illness and duration of extended stay if required.
2. Applicant's valid travel document
3. Sponsor's identity card
Fees:
Your application can be submitted by either yourself or your sponsor. The processing fee is S$20.
